Creating a Form
How to create your own forms and start collecting data
1. Navigate to the Forms section:
- Go to the sidebar and click on Forms
2. Create a new form:
- Click the Create form button
3. Divide your questions into the section
- For example get data for their personal infromation
4. Add your question and select your type
Here is the type that we are offer:
- Short text
- Long text
- Multiple Choice
- Checkboxes
- Number (eg. How many will be serving from your household?)
- Email
- Phone Number
- Date (eg. Your day of birth)
- Time
5. You can create whatever you want and here is the example of a Prayer Request
6. Then publish your form
- Click on Publish and start collecting data!
Share the generated link on your Church Portal on your congregation and gather data!
Tip: You can embed your form into your website!
